MAJOR MEDICAL

Hospitalizations, Surgeries (In or Out-patient), Diagnostic Testing (i.e. x-ray, MRI’s, etc.)

1. Pay your deductible ($1,000, $1,500, $2,500 AND UP)

2. After your deductible is met, the insurance company will pay either 80% or 50% of the expenses until you reach your maximum out of pocket.

3. After you pay both your deductible and co-insurance, the plan pays 100% of the covered expenses for the remainder of the calendar year, all you pay are the required co-pays or service fees.
